This document is a repair work order template. It includes sections for:
- Work order information: work order number, priority level, date issued, assigned technician, and requestor
- Work location: address, building, and room number.
- Work details: request type, equipment/asset ID, physical location, and a description of the work
- Work status: Checkboxes for “Complete” or “Not complete” and a space for technician notes
- Expenses: parts and materials (part number, quantity, amount, totals) and labor (hours, rate, amount, totals)
- Supervisor approval: name, signature, and date
