The graphic design request form includes sections and fields for the following:
- Requestor information: name, phone number, and email.
- Request details: type of design (print, digital, branding, presentations, infographics, motion, packaging/merchandise, or other), project title, dimensions, quantity, content, date of request, desired completion date, urgency (low, medium, high), and project description
- For use by design department: accepted by, date, and order number
